Residential Services of Parker Personal Care Homes

     Currently Parker Personal Care Homes, Inc. provides the following types of residential services: 

Independent Living Services

Host Home Services

Foster Care Services

Group Home Services

Independent Living Services

     Consumers in Independent Living Services live in individual apartments within the community.  These consumers are able to live without direct supervision for much of the day and possess basic independent living and safety skills.  Parker Personal Care Homes, Inc. staff work with consumers several hours each week to assist consumers in developing independent living skills, such as meal planning/preparation, grocery shopping and cleaning.  Staff also assists with medical appointments and social activities.   The number of hours allotted and types of supports provided are determined by each consumer’s interdisciplinary team based on individual needs that are set forth in the Individualized Plan (IP).  PPCH Program Directors oversee the staff working in Independent Living Services and monitor the homes regularly to ensure the services provided are meeting each consumer’s needs appropriately.

Host Home Services

     Host homes are small family-like settings in which a maximum of two consumers live with an independent contractor (Host Home Provider) and are included in the provider’s family and daily life.  The home is generally owned or rented by the Host Home Provider, who is responsible for the overall care and well-being of the consumers in their home, including personal hygiene, health appointments and daily living skills.  Host Home Providers also offer programmatic training and support to assist consumers in becoming as independent as possible.  Consumers living in Host Homes generally require more supervision than those in Independent Living settings.  The types of supports and the level of supervision provided to each consumer is based on individualized needs as determined by the IDT and set forth in the IP.  Parker Personal Care Homes, Inc. monitors each host home on a monthly basis to ensure a safe and hospitable living environment.  Program Directors assigned to each consumer also monitor the home on a regular basis to ensure that the consumer’s needs continue to be met appropriately.

Foster Care Services

     The Foster Care program provides residential services to children under the age of 21.  As with Host Homes, Foster Care homes are small family-like settings in which one or two children live with an Independent Contractor (Foster Care Provider) and are included in the provider’s family and daily life.  The home is generally owned or rented by the Foster Care Provider who is responsible for the overall care and well being of the children in their home including personal hygiene, health appointments and daily living skills.  Foster Care Providers also offer programmatic training and support to assist consumers in becoming as independent as possible.  The Foster Care Program Director monitors each host home on a monthly basis to ensure a safe and hospitable living environment and to ensure that the child’s needs continue to be met appropriately.

Group Home Services

     PPCH, Inc. currently operates one Group Home setting, Kelly’s Place.  The home is owned by PPCH, Inc. and has 24-hour rotating staff, who are employees of PPCH.  Kelly’s Place staff provide full care and supervision to the consumers in the home while promoting a family style living environment.  Staff is responsible for ensuring that basic daily health and safety needs are met, but also ensure that the consumers living here are included in social activities and community outings.  Kelly’s Place is licensed by the State of Colorado, Department of Public Health and Environment.  The Health Facilities Program Director at PPCH routinely monitors Kelly’s Place to supervise the staff and ensure that the needs of the consumers are being met.  At this time, Parker Personal Care Homes, Inc. is at full capacity at Kelly’s Place.

Staffed Three-Bed PCA Services

     PPCH, Inc. also operates one staffed three-bed PCA, Robb House.  This home is owned by PPCH, Inc. and is staffed by PPCH employees.  Staff is onsite 24 hours to provide full care and supervision for the consumers in the home.  The Health Facilities Program Director routinely monitors Robb House to supervise staff and ensure that the needs of the consumers are being met.  At this time, Parker Personal Care Homes, Inc. is at full capacity at Robb House.

All Types of Services

     Parker Personal Care Homes, Inc. Administration provides a 24-hour on call Director to handle urgent and/or emergency situations.  Additionally, Parker Personal Care Homes, Inc. has a Registered Nurse on staff to assist with medical issues.  For more information regarding Parker Personal Care Homes, Inc.
