Program Manager – SUPPORTED LIVING SERVICES

Parker Personal Care Homes is seeking a Program Manager to ensure that meaningful and person centered services are provided to the individuals participating in supported living services. Provides supports to the Program Director(s) to ensure services are being delivered per individual services plans and to help with the day to day operations of the SLS program.  Provide supervision, training and teaching opportunities to SLS Direct Support Professionals so that they implement the SLS services for each individual per service descriptions, rules and regulations

 

Job duties:

  • Implement all consumer Service Plans, Individual Service and Support Plans, and the recommendations of team members

  • Attend team meetings and meet and greets as requested by a Program Director or Director of Children and Family Services.

  • Complete daily review of contact notes entered by DSPs and follow up on any missing or incorrectly entered notes.

  • Provide individual specific training to all new staff and as changes occur and new training is needed. Provide new staff introductions to individuals when new staff begin working with a new individual.

  • Assist with creating new staff schedules and making changes to staff schedules with the support of the Program Director. Ensure that schedules are signed and implemented by the DSPs.

  • Management of Personal Needs Funds (PNF) accounts for individuals with PNF obligations

  • Manage the SLS vehicle fleet by conducting weekly inspections and scheduling service for vehicles as needed. Replenishing vehicle safety supplies for the SLS cars as needed.

  • Ensure accuracy of individual information in Therap by conducting audits as requested by the Director of Children and Family Services.

  • Ongoing communications with supervisors, team members, family members regarding any issues, concerns, problems (i.e. programmatically and medically)

  • Enter new individuals in Therap as requested by the Program Director and ensure all required documentation is in Therap within 30 days of new enrollments.

  • Follow all consumers Service Plans, Individual Service and Support Plans, and the recommendations of team members

  • All duties to be carried out with the utmost professionalism and appropriate manner at all times

  • Observing confidentiality of consumers and PPCH operations

  • Familiar with all PPCH policies/ procedures as well as those outlined by Division for Developmental Disabilities

  • Must have a valid Colorado driver’s license and be able to safely operate a 15 passenger van

  • Must attend and show competency in all PPCH training and meetings

 

Qualifications:

  • High School diploma or equivalent

  • Ability to read and write English proficiently

  • Experience with the Colorado Developmental Disabilities system

  • Knowledge of PPCH policies, procedures, operations and systems

  • Any experience or knowledge of the health care field

  • Working with others in a health care capacity

  • Good oral and written communication skills

  • Knowledge of Microsoft Office components

  • Professional attitude and teamwork mentality

  • Outstanding attendance and ability to self-direct

  • Current/Valid Colorado Driver’s License

  • Good driving record that meets the requirements of agencies insurance carrier

  • Ability to pass criminal background checks

  • Ability to prove authorization to work in the U.S.

 

Minimum Physical Requirements and Working Conditions:

  • This job requires sitting at a desk for up to 8 hours with breaks and ability to walk or stretch as needed, typing and sitting at a computer, walking short distances and driving for no more than one (1) hour stretch of time but may include several trips in one day. 

  • May be required to bend, stoop, squat, and reach overhead and away from body as it pertains to assessing an environment.  

  • Job will also include repetitive motions such as typing and driving and may include lifting a maximum weight of 15lbs. 

  • Must be able to lift approximately 75lbs. 

  • Requires repetitive lifting and carrying of approximately 15lbs as it pertains to consumer care items. 

  • Must be able to push/pull a wheelchair with a consumer of moderate weight. 

  • Requires moderate amount of reaching over head and away from body. 

  • Requires staff to sit for periods of time, walk for up to several hours at a time with breaks. 

  • Requires staff to squat and climb as it pertains to consumer care. 

  • Must be able to operate a vehicle in varying weather conditions. 

  • Must sit in a moving vehicle for long periods of time. 

  • Work is primarily indoors but does require driving and brief exposure to various elements and road conditions. 

  • Must be cognitive and utilize safety skills while walking, driving and maneuvering through various weather conditions. 

  • Daily significant contact with agency personnel and its employees or contractors as well as outside governing agencies and benefit carriers. 

  • Daily significant contact with individuals with Developmental Disabilities.  Daily contact with community members and professionals within the community.
