Program Manager - INSPIRE

Position Description: To provide a residential program that promotes safety and healthy living for the individuals living in the Group Home.  The Program Manager will provide supervision, training and teaching opportunities to Residential Program Counselors so they can provide ongoing assistance and support to the individuals in services. 


Job duties:

  • Must observe the standard job description for Health Facilities Program Counselors

  • Follow all consumers Service Plans, Individual Service and Support Plans, and the recommendations of the team members

  • Attendance at any team meetings as requested

  • Monitoring PC’s to ensure their support matches the Programs philosophies

  • Providing daily documentation, as per PPCH policies and procedures and the rules/ regulations of Division for Developmental Disabilities

  • Review of all consumers files and staff documentation

  • Collect all paperwork from the house weekly and bring to main office

  • Cover vacant shift at the Group Home

  • Ensure all necessary paperwork is on hand at the house

  • All duties are carried out with the upmost professionalism and appropriate manner at all times

  • Ensure the safety and well-being of all consumers by providing proper supervision to staff and instructing/ training as needed

  • Ongoing communications with supervisors, team members, family members regarding any issues, concerns, problems (i.e. programmatically and medically)

  • Maintaining Program Counselor schedules (limiting overtime, ensuring shifts are covered either by available staff or himself/herself, covering on-call situations)

  • Responsible for conducting assessments of consumers, staff, and environment as directed by the Program Director

  • Oversee and maintain Health Facility functions while Program Director is unavailable

  • Oversee and maintain consumer schedules for medical appointments (annual physicals, bi-annual dental, quarterly psychiatry, ER visits as needed, etc.) and ensure proper follow through including transportation as needed

  • Supply checks and ordering as needed (i.e. groceries every week, within budget, DME, medications, etc.).

  • Organize community activities at the house to promote social skills and community participation

  • Observing confidentiality and of consumers and PPCH operations.

  • Review safety checks and communicate any concerns regarding PPCH vehicles

  • Familiar with all PPCH policies/ procedures as well as those outlined by Division for Developmental Disabilities

  • Must be able to safely operate a 15 passenger van

  • Must attend and show competency in all PPCH training and meetings

  • Must abide by and follow the rules and regulations, as set forth by the Division of Intellectual Disabilities.

Qualifications:

  • Experience with the Colorado Developmental Disabilities system

  • Experience with PPCH policies and procedures

  • Must have a high school diploma or equivalent

  • Any experience or knowledge of the health care field

  • Working with others in a health care capacity

  • Ability to read and write English proficiently

  • Good oral and written communication skills

  • Knowledge of Microsoft Office components

  • Professional attitude and teamwork mentality

  • Outstanding attendance and ability to self-direct

  • Current/Valid Colorado Driver’s License

  • Good driving record that meets the requirements of agencies insurance carrier

  • Able to safely operate a 15 passenger van

  • Ability to pass criminal background checks

  • Ability to prove the right to work in the U.S.

 

Minimum Physical Requirements and Working Conditions:

  • Must be able to lift approximately 75lbs. 

  • Requires repetitive lifting and carrying of approximately 15lbs as it pertains to consumer care items and/or office supplies.

  • Must be able to push/pull a wheelchair with a consumer of moderate weight. 

  • Must be able to physically assist consumers with transfers from wheelchair to changing table, toilet or car seat as needed.

  • Requires moderate amount of reaching over head and away from body

  • May be required to bend, stoop, squat and climb as it pertains to assessing an environment and/or to consumer care.

  • Requires staff to sit for periods of time, walk for up to several hours at a time with breaks

  • Requires sitting at a desk for up to 8 hours with breaks and the ability to walk or stretch as needed.

  • May include repetitive motions such as typing and sitting at a computer,

  • Walking short distances

  • Driving for no more than one hour but may include several trips in one day.

  • Work is primarily indoors at the Group Home and out in the community, which does require driving and brief exposure to various elements and road conditions. 

  • Must be able to operate a vehicle in varying weather conditions. 

  • Must be cognitive and utilize safety skills while walking, driving and maneuvering through various weather conditions.

  • Must be able to sit in a moving vehicle for long periods of time.

  • Daily significant contact with agency personnel and its employees or contractors as well as outside governing agencies and benefit carriers. 

  • Daily significant contact with individuals with Developmental Disabilities.  Daily contact with community members and professionals within the community.
