Program Director – SUPPORTED LIVING SERVICES (SLS)
Employment type: full-time / non-exempt

 
Position Description: To provide meaningful, educational and pre-vocational activities to intellectually and developmentally disabled adults. Providing supervision, mentorship and teaching opportunities so that they may learn the skills of daily living. To enhance their choice making and empowering the individuals to make informed decisions about their own life experiences. 


Job duties:

  • Must observe the standard job description for Supported Living Services Managers and Direct Service Professionals

  • Follow all consumers Service Plans, Individual Service and Support Plans, and the recommendations of the team members to ensure quality services are delivered to customers.

  • Provide documentation, as per PPCH policies and procedures and the rules/ regulations of Health Care Policy and Finances, as well as Colorado Department of Public Health and Environment, as applicable.

  • Review of all consumers files and staff documentation

  • All duties are carried out with the upmost professionalism and appropriate manner at all times

  • Ensure the safety and well-being of all consumers by providing proper supervision to staff and instructing/ training as needed.

  • Provide ongoing communication with supervisors, team members, family members regarding any issues, concerns, problems (i.e. programmatically and medically)

  • Responsible for conducting assessments of consumers, staff, and environment as directed by the Director of Children and Family Services.

  • Provides staff supervision, billing for services provided, and staff evaluations.

  • Oversee and maintain Supported Living Services functions.

  • Observe confidentiality and of consumers and PPCH operations.

  • Review safety checks and communicate any concerns regarding PPCH vehicles

  • Maintain compliance with all PPCH policies/ procedures as well as those outlined by Division for Developmental Disabilities

  • Must attend and show competency in all PPCH training and meetings

Qualifications:

  • Must have a Bachelor’s degree Social or Behavioral Sciences, or related field.

  • Must possess a strong knowledge of Social Services and out of home placements, with a particular emphasis on adults with Intellectual and/or Developmental disabilities.

  • Must be able to demonstrate administrative skill and leadership qualities.

  • Experience with job coaching/DVR

  • Knowledge of Therap computer system

  • Ability to read and write English proficiently

  • Experience with the Colorado Developmental Disabilities system

  • Experience with PPCH policies and procedures

  • Any experience or knowledge of the health care field

  • Working with others in a health care capacity

  • Good oral and written communication skills

  • Knowledge of Microsoft Office components

  • Professional attitude and teamwork mentality

  • Outstanding attendance and ability to self-direct

  • Current/Valid Colorado Driver’s License

  • Good driving record that meets the requirements of agencies insurance carrier

  • Able to safely operate a 15 passenger van

  • Ability to pass criminal background checks

  • Ability to prove the right to work in the U.S.

 

Minimum Physical Requirements and Working Conditions:

  • Must be able to lift approximately 50lbs

  • Requires repetitive lifting and carrying of approximately 15lbs as it pertains to consumer care items and/or office supplies.

  • Requires moderate amount of reaching over head and away from body

  • May be required to bend, stoop, squat as it pertains to assessing an environment.

  • Requires sitting at a desk for up to 8 hours with breaks and the ability to walk or stretch as needed.

  • May include repetitive motions such as typing and sitting at a computer, sorting recyclables.

  • Walking short distances

  • Driving for no more than one hour but may include several trips in one day.

  • Requires staff to squat and climb as it pertains to consumer care

  • Work is primarily indoors and out in the community, which does require driving and brief exposure to various elements and road conditions. 

  • Must be able to operate a vehicle in varying weather conditions. 

  • Must be cognitive and utilize safety skills while walking, driving and maneuvering through various weather conditions.

  • Must be able to sit in a moving vehicle for long periods of time.

  • Daily significant contact with agency personnel and its employees or contractors as well as outside governing agencies and benefit carriers. 

  • Daily significant contact with individuals with Developmental Disabilities.  Daily contact with community members and professionals within the community.
