Position Description: To be responsible for oversight of individuals with Developmental Disabilities who reside with and receive services in a Host Home.

Job duties:

  • The Program Director is responsible for the daily operation of a caseload of clients that reside in a Host Home Setting.

  • Accountability for being in compliance with rules and regulations governing Client Residential Services  in the State of Colorado, including compliance with record keeping, bookkeeping and reporting requirements as necessary – includes Division for Developmental Disabilities, Colorado Department of Public Health and Health Care Policy and Financing.

  • Implementation of PPCH policies and procedures.

  • Responsible for ensuring overall professionalism of the agency and its Host Home Program.

  • Ensuring quality assurance of all Host Homes on caseload, including meeting standards of agency and governing bodies. This includes ensuring that documentation remains current, that Host Home Providers remain up to date on programmatic documentation and that visits to the Host Home Providers are conducted regularly.

  • Ensuring satisfaction of clients residing in our Host Home program.

  • Ensure client health and safety is addressed appropriately and in accordance with rules and regulations, as well as client choice surrounding the direction of their lives.

  • Ensures client specific training of all residential Host Home providers and respite providers.

  • Program Development and oversight including program entries into Therap and monthly monitoring of data entry by Host Home Providers.

  • Good communication with families, contractors, staff, team members and PPCH team members.

  • Attends Service Plan meetings and supports clients in development of annual plan that addresses what is important to and important for their upcoming year.

  • Attends IDT’s.

  • Responds to emergency situations in the Host Home.

  • Works proactively to support the ongoing success of the Host Home placement.

  • Oversees Host Home to ensure minimum standards are met in areas of cleanliness and maintenance.

  • May assist with Administrative duties within PPCH office (filing, answering phone calls, etc.).

  • Performs other duties, as assigned, within ability and responsibility level of the job.

  • Bachelor’s degree from an accredited college or university preferred.

  • Must possess a strong knowledge of Social Services and out of home placements, with a particular emphasis on adults with Intellectual and/or Developmental disabilities.

  • Must be able to demonstrate administrative skill and leadership qualities.

  • Ability to read and write English proficiently

  • Experience with the Colorado Developmental Disabilities system

  • Experience or knowledge of the health care field

  • Working with others in a health care capacity

  • Good oral and written communication skills

  • Knowledge of Microsoft Office components

  • Professional attitude and teamwork mentality

  • Outstanding attendance and ability to self-direct

  • Current/Valid Colorado Driver’s License

  • Good driving record that meets the requirements of agencies insurance carrier

  • Ability to pass criminal background checks

  • Ability to prove the right to work in the U.S.


Minimum Physical Requirements and Working Conditions:

  • Requires repetitive lifting and carrying of approximately 15lbs as it pertains to consumer care items and/or office supplies.

  • May be required to bend, stoop, squat as it pertains to assessing an environment.

  • Requires sitting at a desk for up to 8 hours with breaks and the ability to walk or stretch as needed.

  • May include repetitive motions such as typing and sitting at a computer

  • Walking short distances

  • Driving for no more than one hour but may include several trips in one day.

  • Work is primarily indoors and out in the community, which does require driving and brief exposure to various elements and road conditions. 

  • Must be cognitive and utilize safety skills while walking, driving and maneuvering through various weather conditions.

  • Daily significant contact with agency personnel and its employees or contractors, outside governing agencies, individuals with Intellectual and Developmental Disabilities, community members and professionals within the community.
