Position Description: This position will provide management to maintain meaningful, educational and supportive services and direct care to children with developmental disabilities that reside with their family in the family home. 

Job duties:

  • Following all consumers Service Plans, Individual Service and Support Plans, and the recommendations of team members

  • Providing daily documentation, as per PPCH policies and procedures and the rules/ regulations of Division for Developmental Disabilities and Health Care Policy and Financing

  • Ensure the safety and well-being of all consumers by providing proper supervision and assisting as needed

  • Ongoing communications with supervisors, team members, family members families to provide additional support and/or education regarding any issue, concerns, problems

  • Interviewing and hiring new staff members

  • Training new staff members on client/consumer specifics

  • Consulting CCB’s and acting as liaison between resource coordinators and families

  • Scheduling all meet and greets

  • Working with medical staff on client’s protocols

  • All duties are carried out with the utmost professionalism and appropriate manner at all times

  • Provide ongoing assessments of the environment and communicate any concerns or questions immediately to the Associate Executive Director

  • On call, for staff and providing respite for parents as needed

  • Perform behavioral line services

  • Implementing behavioral plans and/or educate parents on behavioral plans 

  • Observing confidentiality of consumers and PPCH operations.

  • Provide supervision and assistance as outlined in the Service Plan or as outlined in agency policies

  • Familiar with all PPCH policies/ procedures as well as those outlined by Division for Developmental Disabilities

  • Must attend and show competency in all PPCH training and meetings

  • Understanding of developmental disabilities 

  • Understanding of early childhood development

  • Previous work with children and families preferred

  • Understanding of positive behavioral intervention techniques 

  • Ability to work independently

  • Ability to work evenings and weekends if needed



  • Bachelor’s degree in related field of study pertaining to Developmental Disabilities, such as education, psychology or social work.

  • Ability to read and write English proficiently

  • Must have hands on experience working with children with Developmental Disabilities

  • Experience with the Colorado Developmental Disabilities system

  • Experience with PPCH policies and procedures

  • Any experience or knowledge of the health care field

  • Working with others in a health care capacity

  • Good oral and written communication skills

  • Knowledge of Microsoft Office components

  • Professional attitude and teamwork mentality

  • Outstanding attendance and ability to self-direct

  • Current/Valid Colorado Driver’s License

  • Good driving record that meets the requirements of agencies insurance carrier

  • Able to safely operate a 15 passenger van

  • Ability to pass criminal background checks

  • Ability to prove the right to work in the U.S.


Minimum Physical Requirements and Working Conditions:

  • Must be able to lift approximately 50lbs 

  • Requires repetitive lifting and carrying of approximately 15lbs as it pertains to consumer care items

  • Must be able to push/pull a wheelchair with a consumer of moderate weight 

  • Requires moderate amount of reaching over head and away from body

  • Requires staff to sit for periods of time, walk for up to several hours at a time with breaks

  • Requires staff to squat and climb as it pertains to consumer care

  • Work is primarily indoors in the clients’ homes and out in the community, which requires exposure to various elements and road conditions 

  • Must be able to operate a vehicle in varying weather conditions 

  • Must be cognitive and utilize safety skills while walking, driving and maneuvering through various weather conditions

  • Daily significant contact with agency personnel and its employees or contractors as well as outside governing agencies and benefit carriers 

  • Daily significant contact with individuals with Developmental Disabilities.  Daily contact with community members and professionals within the community
