     Parker Personal Care Homes, Inc. Day Program is based out of Arvada, Colorado.  Services include Community Accessibility Services & Supports, Non-Community Services & Supports, and Non-integrated work services.

Community Accessibility Services & Supports

     Community Accessibility Services & Supports incorporate time spent in the community to further enhance the individual’s experience in a community setting. 

     The program content for each individual is based upon the needs of each individual receiving services.  The identified needs are achieved through recommendations and consent of the interdisciplinary team of each individual which includes:  the individual, the legal representative (if applicable), family members, resource coordinator, representative from the individual’s residence, program director, as well as any other person that may have involvement and/or interest in the individual’s life.  Service and supports shall be provided pursuant to the Individual Plan and Individuals Service and Support Plans developed by the interdisciplinary team.

   Curriculum & Objectives:  Community Accessibility Services and Supports are designed to provide a variety of activities in the community within a group setting.  These activities are to assist individuals with familiarizing themselves with the community and community resources.  The groups will directly focus on:  academic/functional, hygiene, health and safety, social/recreational, gross motor skills, etc. 

  • A Life Skills Profile and Individual Interest Inventory are conducted for each individual to assess their competencies, wants, needs, preferences and interests.
     

  • Each individual is encouraged to participate within their groups to enhance socialization and building relationships within the community.
     

  • Transportation services that include providing, arranging, and accompanying individuals in order to ensure access to services.
     

  • Assure all medications (prescribed and PRN), and protocols (eating, seizure, G-Tube,
    positioning etc) are provided based on a physician’s signed orders or recommendations.
     

  • Provide opportunities for physical, speech, occupational and recreational therapy based on physician's recommendations.
     

  • Provide meaningful experiences and opportunities for each individual.

Community Accessibility Program Staffing

     Parker Personal Care Homes, Inc. Community Accessibility Program is based on a 1:4 ratio.  Staff is responsible for the implementation and documentation of the individuals they work with during the course of the program.  The day program managers supervise the program counselors.  There are three program managers that are responsible for the overall coordination and implementation of services within the day program setting. There are two program directors. The directors are responsible for developing and implementing the Individualized Service and Support Plans based on the Individualized Plan, supervise the coordination and implementation of therapies and protocols as needed, coordinate training needs of the program counselors, as well as assuring the completion of written reports, communication among the interdisciplinary team and representing Parker Personal Care Homes at team meetings.  The director provides overall supervision and leadership for the day program services.

Non-Community Services & Supports (Base Site)

     Non-Community Integrated services and supports are primarily habilitative in nature with an emphasis on skill development and a focus on generalizing those skills.  Services are provided within a base site location as well as incorporating time spent in the community to further enhance the individual’s experience beyond a sheltered workshop.

     The program content for each individual is based upon the needs of each individual receiving services.  The identified needs are achieved through recommendations and consent of the interdisciplinary team of each individual which includes:  the individual, the legal representative (if applicable), family members, resource coordinator, representative from the individual’s residence, program director, as well as any other person that may have involvement and/or interest in the individual’s life.  Service and supports shall be provided pursuant to the Individual Plan and Individuals service and Support Plans developed by the interdisciplinary team.

The groups will directly focus on:  academic/functional, hygiene, health and safety, social/recreational, gross motor skills, community access etc.

When individuals are in the community, staffing will be 1:2 ratios in order to provide a higher level of supervision and assistance.  The day program managers supervise the program counselors.  There are three program managers that are responsible for the overall coordination and implementation of services within the day program setting. There are two program directors.  The directors are responsible for developing and implementing the Individualized Service and Support Plans based on the Individualized Plan, supervise the coordination and implementation of therapies and protocols as needed, coordinate training needs of the program counselors, as well as assuring the completion of written reports, communication among the interdisciplinary team and representing Parker Personal Care Homes at team meetings.  The director provides overall supervision and leadership for the day program services.

Non-integrated Work Services

     Non-integrated Work Services program provides opportunities for individuals to work in a supported setting.  This program is designed to enhance the individual’s vocational interests and abilities.  The program content for each individual is based upon the needs of each individual receiving services.  The identified needs are achieved through recommendations and consent of the interdisciplinary team of each individual which includes: the individual, the legal representative (if applicable), family members, resource coordinator, representative from the individual’s residence, program director, as well as any other person that may have involvement and/or interest in the individual’s life.  Service and supports shall be provided pursuant to the Individual Plan and Individual’s Service and Supports Plans developed by the interdisciplinary team.

  • Transportation services that include providing, arranging, and accompanying individuals in order to ensure access to the job site.

  • Provide meaningful experiences and opportunities for each individual.

Non-integrated Work Services Staffing

     Parker Personal Care Homes Non-integrated work services Program is based on a 2:5 ratio.  Staff is responsible for the implementation and documentation of the individuals they work with during the course of the program.  The day program managers supervise the program counselors.  There are three program managers that are responsible for the overall coordination and implementation of services within the day program setting. There are two program directors.  The directors are responsible for developing and implementing the Individualized Service and Support Plans based on the Individualized Plan, supervise the coordination and implementation of therapies and protocols as needed, coordinate training needs of the program counselors, as well as assuring the completion of written reports, communication among the interdisciplinary team and representing Parker Personal Care Homes at team meetings.  The director provides overall supervision and leadership for the day program services.
