Child Placement Worker –  FOSTER PPCH/CHILD PLACEMENT AGENCY

Job duties:

  • Must follow the State of Colorado Vol. 7, Rules and Regulating Child Placement Agencies including Chapter 7, General Rules for Child Care Facilities 7.701, Specialized Group Facilities 7.709, Rules Regulating Child Placement Agencies 7.710 and  Quality Standards for 24 Hour 7.714.

  • Provide intensive case management to individual children and their families and coordinate services

  • Ensure full compliance with record keeping, bookkeeping and reporting requirements as necessary and conduct monthly book audits.

  • Communicate and collaborate with the Department of Human Services and submit all required documentation and quarterly reports in a timely manner.

  • Coordinate medical services for the children placed in the care of any foster home, or group home, certified by Parker Personal Care Homes, Inc., ensure that the children attend all necessary appointments and follow up visits.

  • Provide monitoring and support to foster homes and group homes – conduct monthly home visits and environmental studies to assess the ability of foster homes to meet the children’s needs.

  • Attend all staffing, school meetings and treatment team meetings to facilitate the successful collaboration of service providers.

  • Paraprofessionals/trainee may complete home assessments of the physical environment for foster homes for compliance with regulations; act as a liaison with courts, schools, foster parents and all peripheral parties under the direct supervision of a Placement Supervisor or a Placement Worker.

  • The Placement Worker may be called upon to supervise a paraprofessional or trainee in the completion of these duties.

  • May assist with Administrative duties within PPCH office (filing, answering phone calls, etc.)

  • Other duties as assigned, within ability and responsibility level of the job.

 

Qualifications:

  • Must have a Bachelor’s degree in Social or Behavioral Sciences from an accredited college or university and shall be shall be supervised by a qualified Placement Supervisor.

  • If the individual has a Bachelor’s degree in a non-related field, she/ he shall have at least two years full time or equivalent part time experience working in the social services or human services field with children and adolescents, supervised by an individual with a Master of Social Work or other Master’s degree in the social or behavioral sciences. 

  • Worker must possess excellent interpersonal skills and strong organizational skills. 

  • At least two years of full time or equivalent part time experience working in the social

  • Must possess a strong knowledge of Social Services and out of home placements, with a particular emphasis on adults with Intellectual and/or Developmental disabilities.

  • Experience working with various team members and coordination of team

  • Experience working with Mental Illness and/ or other disabilities

  • Ability to read and write English proficiently

  • Good oral and written communication skills

  • Knowledge of Microsoft Office components

  • Professional attitude and teamwork mentality

  • Outstanding attendance and ability to self-direct

  • Current/Valid Colorado Driver’s License

  • Good driving record that meets the requirements of agencies insurance carrier

  • Able to safely operate a 15 passenger van

  • Ability to pass criminal background checks

  • Ability to prove the right to work in the U.S.

 

Minimum Physical Requirements and Working Conditions:

  • Requires repetitive lifting and carrying of approximately 15lbs as it pertains to consumer care items and/or office supplies.

  • Requires moderate amount of reaching over head and away from body

  • May be required to bend, stoop, squat as it pertains to assessing an environment.

  • Requires staff to sit for periods of time, walk for up to several hours at a time with breaks

  • Requires sitting at a desk for up to 8 hours with breaks and the ability to walk or stretch as needed.

  • May include repetitive motions such as typing and sitting at a computer, sorting recyclables.

  • Walking short distances

  • Driving for no more than one hour but may include several trips in one day.

  • Requires staff to squat and climb as it pertains to consumer care

  • Work is primarily indoors and out in the community, which does require driving and brief exposure to various elements and road conditions. 

  • Must be able to operate a vehicle in varying weather conditions. 

  • Must be cognitive and utilize safety skills while walking, driving and maneuvering through various weather conditions.

  • Must be able to sit in a moving vehicle for long periods of time.

  • Daily significant contact with agency personnel and its employees or contractors as well as outside governing agencies and benefit carriers. 

  • Daily significant contact with individuals with Developmental Disabilities, children in the foster care system and team members (i.e. foster families, biological families, DHS workers, therapists).

  • Daily contact with community members and professionals within the community.
